It Was Never Just the Nicotine: What the Throat Hit Studies Found

In one experiment, researchers dripped nicotine straight into smokers' veins. Craving went down. Satisfaction did not move. Then they handed over a cigarette with the nicotine taken out, and the satisfaction came back.

The short version

The experiment that split the puff in two

Jed Rose ran a nicotine research lab at Duke for decades, and one question kept coming back: if nicotine is the whole story, why does nicotine replacement work so much less well than it should?

In 2000 his team tested 80 smokers. Some received nicotine by IV, matched to the dose they would normally get from smoking. Some received saline. Each was tested with and without a denicotinized cigarette. The cigarette with no nicotine cut craving and was rated satisfying and rewarding. The IV nicotine cut craving too, but delivered no satisfaction. Put the two together and the result felt like a regular cigarette.

The drug and the sensation do different jobs. You need to account for both.

Where a puff is felt

Fifteen years earlier, Rose had numbed smokers' mouths and airways with a local anesthetic. The instant relief from a puff disappeared. Later work gave the key sensation a name, the tracheal scratch, and found that satisfaction tracked how strongly people felt it.

The trigeminal nerve carries touch, temperature and irritation signals from the mouth and nose. It is why mint feels cool and pepper feels sharp.

Smoke without nicotine

Several labs then tested cigarettes with the nicotine stripped out. At the US National Institute on Drug Abuse, smokers who had gone 12 hours without tobacco found that denicotinized cigarettes eased withdrawal as much as regular ones. A larger follow-up confirmed with blood tests that no nicotine was delivered, and craving and withdrawal ratings still fell.

The longest test ran 11 days with 30 smokers living in a research unit at Johns Hopkins. Craving relief after each cigarette held up the whole time, with or without nicotine. But the people smoking the nicotine-free version gradually smoked less and worked less hard to earn puffs.

The ritual keeps working for a long time after the drug is gone. It fades, but slowly.

From the lab to a real trial

If the sensation matters, could you replace it with something harmless? Rose's group tried citric acid, the acid in lemons, as a fine mist. In the first test, 15 smokers rated it more satisfying than plain air. A 3-week trial with 74 smokers found that a hand-held version eased craving and low mood.

Then came the real test: a randomized, double-blind trial of 100 pack-a-day smokers. Everyone got the nicotine patch. Half also got the citric acid inhaler, half a placebo inhaler.

Read the footnote on that chart. The inhaler helped people through the first ten weeks, and the more they felt it, the better they did. It did not change where they were at six months. Sensory replacement looks like a tool for getting through the hard stretch, not a cure.

And then there is pepper

In 1994 the same lab gave 48 smokers, fresh off a night without cigarettes, a device to puff on for three hours. One version delivered vapor from black pepper essential oil. One was mint. One was empty. Craving was significantly lower with pepper than with either of the others, and people reported stronger sensations in the chest.

The caveats, stated plainly

  • Nicotine still matters. When smoke-free cigarettes were tested with and without nicotine, the nicotine version relieved withdrawal better. The same held for e-cigarettes.
  • Expectation is part of it. In a study of 128 vapers, what people were told was in the device shaped how much it reduced their craving.
  • Most of these studies are small and measure hours or days. Only a few tracked whether people stayed quit.
  • None of these studies tested CAPNOS. The research devices used smoke, citric acid aerosol or pepper vapor. CAPNOS uses flavored pressurized air.

Rose's own summary, in a 2006 review, was that nicotine replacement will stay useful, and that the next gains will come from tools that take the non-nicotine side of smoking seriously.

Questions people ask

What is a throat hit?

It is the physical sensation at the back of the throat and in the windpipe when you inhale smoke or vapor. Researchers call it airway sensory stimulation, or more bluntly the tracheal scratch. In one study, how satisfying a nicotine-free inhaler felt tracked closely with how strong that sensation was.

Can you get that sensation without nicotine?

Yes. Researchers have produced it with citric acid mist, black pepper vapor and cigarettes with the nicotine removed, and in each case it reduced craving in the lab.

Why does the patch not feel satisfying?

A patch delivers nicotine slowly and with no sensation at all. When researchers delivered nicotine by IV, craving went down but people reported no satisfaction. The feel of the puff seems to supply that part.

Is CAPNOS the same as the inhalers in these studies?

No. The research devices delivered citric acid aerosol, pepper vapor, or smoke from denicotinized tobacco.
